Job Title: Team Lead I-Care Coordinator

FLSA Status: Non-Exempt

Summary:

Provide Care Coordination and Rehabilitative Case Management (RCM) for consumers of Gulf Coast Center (GCC). Primary responsibility will be conducting Activities that are designed to ensure timely access to needed services and supports, facilitate communication among care providers, eliminate duplication of service, ensure uninterrupted service during transitions of care and track progress toward achievement of treatment goals. Address potential gaps in meeting persons served interrelated medical, social, developmental, behavioral, educational, informal support system,and financial needs in order to achieve optimal health and wellness outcomes, according to preferences of the individual served, as well as applicable Legally Authorized Representative (LAR). Additionally, to coordinate and implement Texas Resiliency and Recovery (TRR) RCM as part of an interdisciplinary team. This model incorporates resiliency and recovery-oriented services, supports, practices and beliefs into publicly funded mental health delivery systems. As part of an interdisciplinary team, and within the scope of the team, The Care Coordination Team Lead will be responsible for consultation with other team members on Care Coordination and RCM issues; developing full range of options in the community settings; rehabilitation self-management and wellness strategies; coordinating medical documentation within medication checkups for GCC consumers, and technical assistance as part of the team in an individual and/or group capacity. The CC will be responsible for completion of Adult Needs & Strength Assessments (ANSA), Person Centered Recovery Plans (PCRP), & targeted case management to individuals presenting for behavioral health services, brief interventions such as crisis resolution, skills training and education, case management, and liaison between other programs and organizations. Collect, organize, and submit SUPT data to Data Specialist and Project Director.

The Team Lead I Care Coordinator will be responsible for the following:

Essential Job Duties:

• Assist Direct Supervisor in monitoring monthly performance of Adult Service Provision Targets including regular reviews of client contacts reported in morning meeting.
• Assist Direct Supervisor to complete monthly QA to ensure documentation meets fidelity.
• With the assistance of the Direct Supervisor, create daily schedules for all program staff.
• Ensure Care Coordinators are meeting goals to identify a primary care provider for individuals served.
• Monitoring and reconciliation of referrals through identified resource tools and documentation in SmartCare.
• Ensure persons served are connected and engaged with services; ensure individual/family choice is facilitated.
• Serve as a conduit between persons served, clinical staff, and community providers to ensure care is provided, documented, and communicated.

• Facilitate internal and external collaboration to develop person-centered or family-centered integrated recovery plans.
• Foster successful care transitions and transitions over time; organize care activities and share information among all to achieve safer and more effective care.
• Conduct comprehensive and timely activities to resolve barriers to accessing care determined necessary to address unmet needs.
• Organize care activities and share information among all the participants concerned with an individual's care to achieve safer and more effective care.
• Relationship building with community partners and referral sources to ensure access to the right care at the right time for persons served.
• Coordinate access to care via initial screening for admission eligibility and assess need for routine or crisis related services for youth and/or adult services.
• Support provider workflow and patient care by coordinating with department staff regarding financial, benefit, and medical services.
• Conduct the Adult Needs and Strengths Assessment (ANSA) every 6 months and/or Child and Adolescent Needs and Strengths Assessment (CANS) every 90 days or as clinically indicated for individuals as required and assigned. Responsible for ensuring the completion of the initial and annual ANSA/CANS recertification pursuant to the Praed Foundation and HHSC contract. Any lapse in certification will result in formal disciplinary action. Exception: If certification lapse/expiration is reported to supervisor within 24 hours of expiration date, employee will be granted opportunity to recertify by close of following business day. Any lapse/expiration greater than 24 hours will result in termination of employment. Additionally, all certified ANSA/CANS users are required to participate in semi-annual quality assurance training activities conducted by identified ANSA/CANS Super User.

• Specific to working with adults, utilize a variety of skills training methods in conjunction with the IMR (Illness Management and Recovery Model) and may utilize the following intervention strategies: instruction, positive feedback, repetition, role play, behavioral reversal, in-vivo practice and coaching as it relates to an individual's ability to obtain, maintain, or retain integrated employment and/or housing of their choice.
• Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.

• May be required to travel within Brazoria and Galveston Counties.
• Out of region travel may be required to attend trainings.
• Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
• Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

• Requires a bachelor's degree or equivalent in the behavioral sciences field of study from an accredited college or university and/or specialized QMHP certification.

• Two years full-time experience in human services preferred.

• General knowledge of mental health disorders and navigating Electronic Health Records.
• Excellent verbal and written communication skills with knowledge to teach and engage in basic problem-solving strategies to support self-directed recovery.
• Knowledgeable in trauma informed care and community resources.
• Ability to express self clearly and effectively, orally and in writing.
• Requires ability to work within a team approach.
• Requires proficiency with computer skills, programs, and email.

• May require varied work hours depending on need of individuals.
• May require travel to various locations within Brazoria and Galveston Counties.
• Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

• Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
